Frequently Asked Questions

  1. Do you take insurance?

    • I am currently In Network with:

      • Providence

      • PacificSource

      • United Healthcare

      • Moda

      • First Choice/LifeStance

      • Trillium

    • I do NOT take Blue Cross Blue Shield or Kaiser but if you are interested in services you may pay out of pocket and submit paperwork to your insurance for reimbursement.

    • I am also taking fee-for-service clients, meaning the client pays a set price out of pocket for each visit. I can provide you with the required information to submit to your insurance company if you would like to file an out of network claim.

  2. What are your fees?

    • Fees vary and can range between $100 and $425 which is why I am happy to discuss my current fees and payment options. My goal is to be completely transparent by ensuring you understand exactly what you will be charged and why.

  3. What ages do you treat?

    • At this time I am only accepting ages 18 and up.

  4. What type of disorders do you treat?

    • I can treat a wide range of mental health and health related issues including, but not limited to: depression, anxiety, insomnia, bipolar disorder, PTSD, general fatigue and malaise, and irritability/anger issues. I unfortunately do not specialize in treating ADHD though.

  5. What type of treatment do you offer?

    • I believe the best way to treat mental health is through a holistic view. I offer a combination of treatments including medications, supplements, and lifestyle recommendations. I also use pharmacogenetic testing to assist in medication selection.

  6. Do you offer therapy?

    • I do not offer therapy at this time although there always is a therapeutic component to our sessions.

  7. Do you prescribe controlled substances? â€‹

    • On occasion I do prescribe controlled substances but my ultimate goal is to assist you in improving your quality of life with the minimum amount of medication as possible. Please note if you are prescribed a controlled substance you will be asked to sign a controlled substance agreement and submit to randomized drug screens.

  8. I need a refill for my medication, what should I do?

    • Please sign into the patient portal and send a message with the name of the medication and the pharmacy you wish the refill to be sent to. Although I attempt to respond to requests the same day please allow up to 3 business days for refill requests. If it is urgent you may ask your pharmacy if they will provide a 3 day supply of the medication, although not all pharmacies offer this and it does not include controlled substances.

  9. What is your cancellation/missed appointment policy?

    • I require a notice of at least 2 business days before your appointment which you can do by calling the office or messaging me through the patient portal. You may miss or late cancel up to 3 appointments in a rolling calendar year. The first missed or late cancellation is free, the second is $50, and from then on you will be charged the full fee. It will be your responsibility to pay the fee and you may be asked to keep a card on file in the event of a cancellation/missed appointment. If you are under Medicaid then different terms will apply based on your insurance, which will be discussed at the first visit. Please be aware this policy is strictly enforced as there is a high need for appointments currently.

  10. Do you complete disability or any other type of paperwork?

    • You may request your records but I am not able to complete disability paperwork at this time. There may be a small fee for any paperwork that needs to be completed and it may take up to 1 week for it to be returned.

  11. I'm in crisis what should I do?

    • If you need a response within 2 business days or you are in crisis you can contact the Suicide Prevention Hotline (1-800-273-TALK), White Bird Clinic (541-687-4000), or go to the local emergency department.

  12. How long are the appointments?

    • Initial appointments are typically 60 mins long and follow ups are between 15-30 mins long.

  13. I would like to request an appointment.

    • That's great! Please call the office at 541-777-8879. We will send you paperwork to complete and schedule an appointment. The paperwork must be completed 1 week prior to the first appointment. I want to make the most out of your first visit and having the paperwork ahead of time gives me a chance to focus on you.
